Contact the applicable offices listed in the <a href=\"#offices\">Reporting Major Accidents table<\/a>.<\/p>\n<p>For all other incidents,<b> supervisors must report work-related injuries and \u00a0illnesses within 24 hours of occurrence\u00a0<\/b>(see <a href=\"#accidents\">Accidents\/Illnesses<\/a>).<\/p>\n<p><strong>Exception:<\/strong> Supervisors must report any accidental injury or work-related illness affecting a swing- or graveyard-shift employee <b>within 48 hours<\/b> of occurrence.<\/p>\n<h4><a name=\"accidents\"><\/a>Accidents\/Illnesses<\/h4>\n<p>Accidental injury and work-related illness reporting includes the following:<\/p>\n<ul>\n<li>Notification of emergency medical assistance when required; telephone 911.<\/li>\n<li>Immediate notification of appropriate University offices when a major accident occurs (see <a href=\"#major\">Major Accidents<\/a> and the <a href=\"#offices\">Reporting Major Accidents table<\/a>).<\/li>\n<li>Submission of an online Incident Report by the responsible supervisor within 24 hours (or within 48 hours for a swing- or graveyard-shift employee). See <a href=\"#form\">Incident Report Form Completion<\/a>.<\/li>\n<li>Notification by the supervisor to the unit&#8217;s top administrator and the appropriate safety committee chair of all major accidents.<\/li>\n<\/ul>\n<h3><a name=\"major\"><\/a>Major Accidents<\/h3>\n<p><b>Immediately<\/b> report all major accidents affecting WSU personnel or nonemployees to the offices indicated in the <a href=\"#offices\">Reporting Major Accidents table<\/a>. (See also <a href=\"#nonemployee\">Nonemployee Injuries<\/a> regarding nonemployees.)<\/p>\n<p><strong>Note:<\/strong> If unable to contact personnel at the appropriate University office (see the <a href=\"#offices\">Reporting Major Accidents table<\/a>), contact WSU Police Services; telephone 509-335-8548.<\/p>\n<p>The offices indicated in the table below investigate major accidents and notify other appropriate University units and\/or request assistance as needed. See <a href=\"https:\/\/policies.wsu.edu\/prf\/index\/manuals\/safety-policies-procedures-manual\/sppm-2-26\/\">SPPM 2.26<\/a> for investigation procedures.<\/p>\n<h4>Non-Pullman Locations, All Injured Persons<\/h4>\n<p>The local Environmental Health and Safety (EH&amp;S) unit takes the lead in major accident investigations at non-Pullman locations and coordinates with appropriate WSU Pullman departments (see the <a href=\"#offices\">Reporting Major Accidents table<\/a>). telephone 509-335-1760.<\/p>\n<h5>More Than One Work Shift<\/h5>\n<p>Send a copy of the Leave Report or Time Report to HRS when time loss exceeds one work shift.<\/p>\n<h4><a name=\"investigation\"><\/a>Supervisor&#8217;s Accident Investigation Report<\/h4>\n<p>In addition to submitting the online Incident Report, the supervisor is to complete a <a href=\"http:\/\/policies.wsu.edu\/prf\/documents\/2017\/10\/2-26-supervisors-accident-investigation-report.pdf\">Supervisor&#8217;s Accident Investigation Report<\/a>\u00a0when:<\/p>\n<ul>\n<li>Employee receives medical treatment.<\/li>\n<\/ul>\n<ul>\n<li>Employee is unable to work the next full or subsequent shift(s) as a result of an injury or work-related illness.<\/li>\n<\/ul>\n<ul>\n<li>Events and conditions related to a near miss or minor accident indicate that a potentially serious injury or illness could result from a similar situation.<\/li>\n<\/ul>\n<p>See <a href=\"https:\/\/policies.wsu.edu\/prf\/index\/manuals\/safety-policies-procedures-manual\/sppm-2-26\/\">SPPM 2.26<\/a> for investigation procedures.<\/p>\n<h3>Motor Vehicle Accidents<\/h3>\n<p>For an accident involving a motor vehicle, regardless of how minor, the driver must <b>immediately<\/b> notify his or her supervisor, Risk Management Services, and the Motor Pool (if a Motor Pool vehicle was involved). See <a href=\"https:\/\/policies.wsu.edu\/prf\/index\/manuals\/safety-policies-procedures-manual\/sppm-7-20\/\">SPPM 7.20<\/a>.<\/p>\n<p>For an accident involving a motor vehicle with injuries, the driver must <b>also immediately<\/b> notify the local area law enforcement department. Law enforcement personnel should investigate all accidents resulting in any of the following:<\/p>\n<ul>\n<li>Damage costing over $1000 to motor vehicles<\/li>\n<li>Damage to other property<\/li>\n<li>Injuries to individuals<\/li>\n<\/ul>\n<p>The driver must submit a completed Vehicle Accident Report to Risk Management Services <b>within two working days<\/b> (see <a href=\"https:\/\/policies.wsu.edu\/prf\/index\/manuals\/safety-policies-procedures-manual\/sppm-7-20\/\">SPPM 7.20<\/a>).<\/p>\n<h3><a name=\"nonemployee\"><\/a>Nonemployee Injuries<\/h3>\n<p>University departments coordinating the use of University facilities or University-sponsored activities are responsible for reporting all injuries incurred by individuals who are not employed by WSU. Risk Management Services notifies the state of Washington&#8217;s Torts Claims Division of nonemployee injuries when required. The injured nonemployee may file a claim for damages against the state of Washington (<a href=\"http:\/\/apps.leg.wa.gov\/RCW\/default.aspx?cite=4.92\">RCW 4.92<\/a>). Claim forms and instructions are available from Risk Management Services.<\/p>\n<h4>Nonemployee Students Injuries<\/h4>\n<p>For any injuries to nonemployee students, the responsible University department must submit a completed Incident Report to HRS <b>within 24 hours<\/b>\u00a0(see <a href=\"#form\">Incident Report Form Completion<\/a>). Human Resource Services\u00a0forwards a copy of the completed Incident Report to the Dean of Students office upon submission.<\/p>\n<h4>Volunteers<\/h4>\n<p>For injuries to volunteers, the responsible University department must submit a completed Incident Report to HRS <b>within 24 hours<\/b>\u00a0(see <a href=\"#form\">Incident Report Form Completion<\/a>).<\/p>\n<h4>Visitors<\/h4>\n<p>For injuries to visitors, the responsible University department must submit a completed Incident Report to HRS <b>within 24 hours<\/b>\u00a0(see <a href=\"#form\">Incident Report Form Completion<\/a>).<\/p>\n<h4>Federal Employees<\/h4>\n<p>For injuries to federal employees, e.g., USDA-ARS, follow the procedures outlined in OOL (Office of Legal Counsel) Form CA-10: What a Federal Employee Should Do When Injured at Work. This form indicates actions a federal employee should perform after being injured at work.<\/p>\n<p>For more information, see the <a href=\"http:\/\/www.dol.gov\">U.S. Department of Labor<\/a>.<\/p>\n<p>CA-10 forms are available at applicable USDA-ARS units.<\/p>\n<h3><a name=\"form\"><\/a>Incident Report Form Completion<\/h3>\n<p>The responsible supervisor is to complete an online Incident Report, <b>within 24 hours<\/b> (or within 48 hours for a swing- or graveyard-shift employee), to report <b>any<\/b> accident, injury, work-related illness, or workplace fatality that results from participation in any of the following activities:<\/p>\n<ul>\n<li>University employment<\/li>\n<li>Use of University facilities<\/li>\n<li>University-sponsored activities<\/li>\n<\/ul>\n<h4>Accessing the Form<\/h4>\n<p>To access the online Incident Report system, go to the <a href=\"http:\/\/ihr.hrs.wsu.edu\/forms\/incident_report.aspx\">HRS Incident Report<\/a>.<\/p>\n<p>Enter all pertinent information as prompted at each screen.<\/p>\n<h4>Verifying Circumstances<\/h4>\n<p>The supervisor verifies the actual circumstances of the incident by interviewing the injured person, witnesses, and other involved individuals.<\/p>\n<h4>Amending a Report<\/h4>\n<p>To amend a previously-submitted Incident Report, complete and submit another report. Include the employee&#8217;s name, date of accident, and the new or updated information. Enter the word &#8220;Amend&#8221; in the <b>Complete Description of the Incident<\/b> field.<\/p>\n<h4>Questions<\/h4>\n<p>Address questions regarding the Incident Report to EH&amp;S; telephone 509-335-3041, HRS; telephone 509-335-4521, or Risk Management Services; telephone 509-335-6893.<\/p>\n<h4>Attachments<\/h4>\n<p>To submit any attachments, print a copy of the submitted Incident Report form. Attach the additional documents as needed. See <a href=\"#routing\">Routing the Incident Report and Attachments<\/a> for routing instructions.<\/p>\n<p>To load and print a previously submitted Incident Report, select <b>Load a Saved Form<\/b> from the initial Incident Report screen.<\/p>\n<h5>Witness\/Injured Person Statement (Optional)<\/h5>\n<p>If an injured person wishes to document an incident he or she may elect to complete and submit a Witness\/Injured Person Statement form. Print the master <a href=\"http:\/\/policies.wsu.edu\/prf\/documents\/2017\/10\/2-26-witness-injured-person-statement.pdf\">Witness\/Injured Person Statement<\/a>\u00a0to obtain supplies of the statement form.<\/p>\n<p>The supervisor submits copies of this form to HRS and references the Incident Report submitted to HRS and the department&#8217;s safety committee.<\/p>\n<h5>Supervisor&#8217;s Accident Investigation Report<\/h5>\n<p>The supervisor is to attach a completed Supervisor&#8217;s Accident Investigation Report\u00a0when applicable (see <a href=\"#investigation\">Supervisor&#8217;s Accident Investigation Report<\/a>).<\/p>\n<h4>Workers&#8217; Compensation<\/h4>\n<p><strong>Note:<\/strong> Submittal of the Incident Report does not constitute the filing of a claim for Workers&#8217; Compensation benefits. Refer to <a href=\"https:\/\/policies.wsu.edu\/prf\/index\/manuals\/safety-policies-procedures-manual\/sppm-2-30\/\">SPPM 2.30<\/a> for Workers&#8217; Compensation claim procedures.<\/p>\n<h4><a name=\"routing\"><\/a>Routing the Incident Report and Attachments<\/h4>\n<p>Upon submission, the online system sends copies of the Incident Report form to HRS, EH&amp;S, and the reporting supervisor. If the injured party is a student, the system also sends a copy of the report to the Dean of Students office.<\/p>\n<h5>Risk Management<\/h5>\n<p>If applicable, HRS forwards a copy of the Incident Report to Risk Management Services.<\/p>\n<h5>Human Resource Services (HRS)<\/h5>\n<p>If applicable, the supervisor routes a Supervisor&#8217;s Accident Investigation Report and any attachments to HRS. (See <a href=\"#investigation\">Supervisor&#8217;s Accident Investigation Report<\/a>.) Reference the date of the Incident Report and the name of the injured party on all attachments.<\/p>\n<h5>Environmental Health and Safety (EH&amp;S)<\/h5>\n<p>HRS retains the originals and forwards copies of the attachments to EH&amp;S.<\/p>\n<h5>Safety Committee<\/h5>\n<p>The department prints and routes a copy of the Incident Report with any attachments to the department&#8217;s safety committee representative.
