Departments must complete and affix Item Evaluation forms in order for Surplus Stores to accept the items.<\/p>\n<h4 id=\"One_1\">1.1   Completing the Item Evaluation<\/h4>\n<p>The department must list the following information on an Item Evaluation form prior to sending a biohazard-contaminated item to Surplus Stores:<\/p>\n<ul>\n<li>Disinfectant used for decontamination<\/li>\n<li>Active ingredient in the disinfectant<\/li>\n<li>Method of disinfectant application<\/li>\n<li>Length of time disinfectant was applied<\/li>\n<li>Biological hazard causing the contamination<\/li>\n<\/ul>\n<p>For more information, see the Biosafety Office website at:<\/p>\n<p style=\"padding-left: 30px\"><a href=\"http:\/\/www.bio-safety.wsu.edu\/biosafety\/\">bio-safety.wsu.edu\/biosafety\/<\/a><\/p>\n<p style=\"padding-left: 30px\">For a definition of potential biological hazards:<\/p>\n<p style=\"padding-left: 60px\">Select <strong>Potentially Biohazardous Material<\/strong>.<\/p>\n<p style=\"padding-left: 30px\">For a list of potential biological hazards:<\/p>\n<p style=\"padding-left: 60px\">Select <strong>Laboratory Biosafety<\/strong>, then<br>\nSelect <strong>Classification by Risk Group<\/strong>.<\/p>\n<p>Complete and\/or print the Item Evaluation. Attach the completed form to the item prior to surplus collection.<\/p>\n<h5 id=\"One_1_a\">1.1.a   Departmental Biosafety Manual<\/h5>\n<p>To work with a potentially biohazardous material, the department must develop a biosafety manual that is specific to the work area. The department is to submit the manual draft to the WSU Biological Safety Officer (BSO) for approval.<\/p>\n<p>Links to the biosafety guidelines and the University&#8217;s Biosafety Manual Template are available at the WSU Biosafety website at:<\/p>\n<p style=\"padding-left: 30px\"><a href=\"http:\/\/www.bio-safety.wsu.edu\/biosafety\/\">bio-safety.wsu.edu\/biosafety\/<\/a><\/p>\n<p style=\"padding-left: 30px\">Select <strong>Forms and Templates<\/strong>.<\/p>\n<h4 id=\"One_2\">1.2   Biohazard Decontamination<\/h4>\n<p>Review the appropriate method for decontamination identified in the BSO-approved biosafety manual, Environmental Health and Safety (EH&amp;S) approved Bloodborne Pathogen Exposure Control Plan, or the standard operating procedure (SOP) for the work area. See the list of common biological agents and disinfection procedures in the Emergency Response web training available from the WSU Biosafety website at:<\/p>\n<p style=\"padding-left: 30px\"><a href=\"http:\/\/www.bio-safety.wsu.edu\/biosafety\/\">bio-safety.wsu.edu\/biosafety\/<\/a><\/p>\n<p style=\"padding-left: 30px\">Select Laboratory Biosafety, then<br>\nSelect <strong>Training<\/strong>, then<br>\nSelect <strong>Emergency Response<\/strong>.<\/p>\n<p>For more information, contact EH&amp;S; telephone 509-335-3041; or the Biological Safety Officer; telephone 509-335-4462.<\/p>\n<p>Departmental personnel must wear gloves, eye, and possibly respiratory protection whenever chemical disinfectants are used.
