If damages are $1,000 or more or if bodily injury has occurred, the officer must file a Washington State Vehicle Collision Report (WSP-161). Both forms are provided by law enforcement agencies within the state of Washington.<\/p>\n<h4 id=\"Two_3\">2.3\u00a0\u00a0\u00a0 Obtain Information<\/h4>\n<p>Obtain the following information from other involved parties and witnesses:<\/p>\n<ul>\n<li>Names<\/li>\n<li>Addresses<\/li>\n<li>Telephone numbers<\/li>\n<li>Vehicle descriptions<\/li>\n<li>License numbers<\/li>\n<li>Insurance companies of third parties (names, addresses, telephone numbers)<\/li>\n<\/ul>\n<h4 id=\"Two_4\">2.4\u00a0\u00a0\u00a0 Notify WSU<\/h4>\n<p>Immediately notify the following WSU officials or offices, as appropriate:<\/p>\n<ul>\n<li>Driver\u2019s immediate supervisor<\/li>\n<li>Risk Management; telephone 509-335-6893; e-mail\u00a0<a href=\"mailto:riskmanagement@wsu.edu\">compliance.risk@wsu.edu<\/a><\/li>\n<li>Motor Pool of Facilities Services, Operations if a Motor Pool vehicle is involved; telephone 509-335-9085<\/li>\n<\/ul>\n<p>In the event the offices above are closed, such as holidays or weekends, report the event the following business day.<\/p>\n<h3 id=\"Three_0\">3.0\u00a0\u00a0 Collision Report<\/h3>\n<p>Drivers report all traffic accidents,\u00a0<strong>regardless of how minor, <\/strong><a href=\"https:\/\/des.wa.gov\/policies-legal\/risk-management\/reporting-accident-involving-state-driver\">Vehicle Collision Report<\/a>\u00a0on the State of Washington\u00a0Vehicle Collision Report\u00a0(SF-137).<\/p>\n<p><strong>Note:<\/strong>\u00a0Paper versions of the SF-137 form may be accepted by Risk Management only in rare circumstances.<\/p>\n<p>Within two working days, the driver completes the online SF-137 form and provides a PDF copy of the completed form to their supervisor. The supervisor then e-mails a copy of the completed report to Risk Management; email <a href=\"mailto:compliance.risk@wsu.edu\">compliance.risk@wsu.edu<\/a>.<strong>\u00a0<\/strong><\/p>\n<p>A hardcopy of the SF-137 form is located in the glove compartment of University vehicles for drivers to use as a template for capturing necessary information.<\/p>\n<p><strong>Important!<\/strong>\u00a0Departments should also maintain a supply of SF-137 forms in all department-owned or leased vehicles to be used as templates.<\/p>\n<h4 id=\"Three_1\">3.1\u00a0\u00a0\u00a0 Bodily Injury<\/h4>\n<p>In the event of bodily injury to WSU employees, they are to also complete a <a href=\"http:\/\/ihr.hrs.wsu.edu\/forms\/incident_report.aspx\">HRS Incident Report<\/a> as indicated in\u00a0<a href=\"https:\/\/policies.wsu.edu\/prf\/index\/manuals\/safety-policies-procedures-manual\/sppm-2-24\/\">SPPM 2.24<\/a>\u00a0and\u00a0<a href=\"https:\/\/policies.wsu.edu\/prf\/index\/manuals\/safety-policies-procedures-manual\/sppm-2-30\/\">2.30<\/a>.<strong>\u00a0<\/strong><\/p>\n<h3 id=\"Four_0\">4.0\u00a0\u00a0 Repair Estimates\u00a0<\/h3>\n<h4 id=\"Four_1\">4.1\u00a0 \u00a0 \u00a0WSU Pullman Motor Pool Vehicles<\/h4>\n<p>Vehicles managed by WSU Pullman Motor Pool that need repair should be returned to Motor Pool as soon as possible. Motor Pool and Risk Management then develop and coordinate a repair estimate. The driver\u2019s supervisor submits the Vehicle Collision Report (SF-137) to Risk Management. (See <a href=\"#Three_0\">Section 3.0<\/a>.)<\/p>\n<h4 id=\"Four_2\">4.2\u00a0 \u00a0 \u00a0All Other University Vehicles<\/h4>\n<p>As soon as possible, the driver obtains two written estimates for repairs to the University vehicle. The driver\u2019s supervisor sends the estimates with the SF-137 to Risk Management.<\/p>\n<h4 id=\"Four_3\">4.3\u00a0 \u00a0 \u00a0Glass Repair<\/h4>\n<p>WSU Pullman units are to obtain glass repairs from the Motor Pool. Units in other locations obtain two written estimates from local glass repair shops and submit them with the SF-137 to Risk Management. <strong>Note:<\/strong> Only one estimate is needed if using a vendor under the State\u2019s contract, contact Risk Management for current vendor information.<strong>\u00a0<\/strong><\/p>\n<h4 id=\"Four_4\">4.4\u00a0 \u00a0 \u00a0Auto Body<\/h4>\n<p>Local vendors are to be used for auto body repairs.<\/p>\n<h4 id=\"Four_5\">4.5\u00a0 \u00a0 \u00a0Review of Estimates<\/h4>\n<p>Risk Management reviews the estimates and selects a vendor to provide the repairs.<strong>\u00a0<\/strong><\/p>\n<h3 id=\"Five_0\">5.0\u00a0\u00a0 Claims<\/h3>\n<p>A University traveler involved in an accident while driving a privately-owned vehicle on University business is not reimbursed for deductibles and the traveler\u2019s insurance is primary. (See <a href=\"https:\/\/policies.wsu.edu\/prf\/index\/manuals\/business-policies-and-procedures-manual\/bppm-95-11\/\">UPPM 95.11<\/a>.)<\/p>\n<h4 id=\"Five_1\">5.1\u00a0 \u00a0 \u00a0Third-Party Claims<\/h4>\n<p>A third party who wishes to file a claim against the state of Washington due to an accident with a University vehicle and\/or driver completes a Standard Tort Claim Form (SF-210).
