This SPPM revision includes the following changes:

SPPM 2.16 Safety Orientation Checklist form

This revision adds two new fields, WSU ID and Supervisor/Advisor, to the Safety Orientation Checklist. No changes are made to the accompanying section SPPM 2.16.
