1.0 Reporting Hazards
University personnel observing serious safety concerns affecting or potentially affecting University employees are to immediately contact their supervisor, the unit administrator, and/or the campus Environmental Health and Safety (EHS):

Personnel at WSU locations not listed above may contact WSU Pullman EHS for assistance.
WSU employees must play an active role in creating a safe and healthy workplace and may refuse to perform dangerous tasks with inadequate hazard controls.
Personnel are to report all safety hazards to the unit’s administrator, safety committee, and campus EHS by telephone, e-mail, or using the applicable hazard notification/reporting form. Urgent hazards not requiring an emergency response i.e. 911 must be reported by telephone. Reports must include the location and a description of the hazard:

The unit responsible to address the safety hazard completes the Corrective Action or Subsequent Action section of the applicable hazard reporting form.
2.0 Hazard Corrective or Subsequent Action
The appropriate unit develops and implements corrective or subsequent action to address the hazard. The unit documents the corrective action on the applicable hazard notification form.
3.0 Reporting Hazards Caused by Contractors
If University personnel observe serious hazards to University employees created by building or construction contractors, the personnel are to report the hazards to the applicable campus EHS. See Section 1 for reporting contacts.
The campus EHS coordinates with the campus Facilities Services to communicate the safety concern to the contractor.
